Norfolk Community Health and Care NHS Trust (NCHC) host the Workforce workstream of the STP which seeks to align workforce transformation to the emerging needs of our local population. Health Coaching is a clinical and communication tool which encourages positive behavioral change and increases patient activation allowing clinician and patient to engage more effectively. This engagement creates an environment where patients are more likely to feel empowered, more confident and able to take greater responsibility in managing their own health conditions. As such, the Norfolk and Waveney Health and Care Partnership would like to increase the benefit of Health Coaching to staff and service users by delivering them across the footprint of the STP.
