It is anticipated that the contract will commence on the 1st April 2023 for a period of three (3) years with an option to extend for two (2) further periods of 12 months.The aim of the Service is: (1) To provide an enhanced, personalised transfer of care pathway from hospital to home for Individuals who are recovering from a period of ill health or who need a further period of convalescence and reablement whilst their long-term support needs are assessed.(2)To provide a step-up service to support people who have had a period of ill health or crisis within the community and require convalescence and reablement until they can return to their own homes.
