## Notice

The Primary Care Streaming Service (PCSS) will be provided from the Kettering General Hospital site located within the Emergency Department (ED) The key requirements for the PCSS are: * Provide the right care, by the most appropriate person, in the right setting ensuring consistent responses at the first point of contact. * Streaming non-urgent patients that attend the Emergency Department back to primary care. * Primary care GP working closely with their ED colleagues to collaboratively lead the provision of Primary Care Services at KGH * Access for walk-in patients will be via the Clinical Streamer if appropriate providing a seamless pathway to any further assessment required including ED and referral (if necessary) to a hospital specialist. * Contributing to the Service development of Frail Elderly attendances at the ED from both the admission avoidance and discharge facilitation perspective. * Contributing to the development of ambulance patient access to the service. * Facilitation of unregistered patients to register with a GP practice * Providing health promotion and self-management education to patients, relatives and carers. * Assuring facilitation of 1st Assessment completion within the designated 15 minutes, including NEWS and ECG where required * Seeing and treating and discharging all patients within 4 hours of their arrival at the service * Making all see and treat decisions within 60 minutes of the patient's arrival at the service * Ensuring that necessary transfers occur within 60 minutes of arrival at the service * Assuring completion of Medway for patients attending the service, with appropriate coding
