## Notice

Cambridgeshire County Council (CCC) and Peterborough City Council (PCC) are planning to design a new step-up, step-down care pathway incorporating a range of short term services aimed at supporting people to remain independent in their own homes wherever possible. We anticipate this will include a mixture of short-stay beds, reablement beds and homecare provision. Currently, we work alongside our health colleagues in meeting the requirements for Discharge to Assess. It has not yet been decided if we will be commissioning future provision jointly with Health. Bed based provision will incorporate: * Individuals being discharged from an acute hospital setting who cannot return home immediately but instead need a short period of support in a care setting. * Individuals who are too unwell to be supported at home but who do not need an acute hospital setting * Individuals who are needing a period of planned respite * Individuals who need a period of unplanned/emergency respite Home based provision will incorporate: * Individuals who are being discharged from hospital where it seems probable that they will need a long term care package, but no such package is available at the point of discharge * Individuals in the community whose physical or cognitive condition needs to be met through a long-term care package, but no such care package is available immediately. Views are sought as to the shape this new pathway should take and how it can be delivered. We would like you to share your experiences of delivery of such services, share examples of effective and innovative models you are aware of or have delivered, and also tell us what in your view needs to be in place to help us develop the most effective and innovative model we can. We also want to understand the market appetite for delivering this requirement to inform our procurement and contracting approach and gain maximum interest. It is anticipated that contracts for the new pathway will be in place in November 2021. Currently, there are 18 Interim beds in CCC. Respite beds in CCC will fall under a different contract from 01/04/2021 and are therefore not being included in the development of a new care pathway. There are 4 Interim beds, 2 respite beds and 9 Reablement flats currently commissioned by PCC. In CCC there is also currently domiciliary care provision via a number of Discharge Cars which deliver care to people leaving hospital where a permanent care package cannot be sourced immediately. There are currently 20 cars delivering 1771 hours of care per... The current indicative spend is as follows: PCC Spot and block IRR beds 2019/20 PS1,126,905 CCC Spot and block interim beds only 2019/20 PS1,508.615 CCC Block cars 2020/21 PS1,919,511 There are a number of issues with the current provision, including the need to spot purchase beds as we do not have the correct mix of block bed types, people remaining with the block cars for longer than the anticipated 6 weeks as there is no capacity in the mainstream domiciliary care market, and a lack of therapy support to the IRR beds. The new care pathway needs to address these.
