The Social Prescribing Service is a single point of access that identifies and supports eligible patients with multiple social, emotional and practical needs. The aims and objectives of the service are to: * Improve wellbeing and ability to self-care. * Reduce inappropriate appointments and use of other services. * Increase the number of patients. * Actively identify individuals in the specified categories who will benefit from the service. The successful provider will be expected to: * Triage patients to Link Workers. * Provide assessment of needs i.e. motivational support, goal-setting and referrals to other suitable services. * Develop a Personal Care Plan (PCP). * Provide consultation, wrap around support for patients i.e. be-friending, activities, exercise classes, benefits assessments, housing, employment, training, arts, creative activities, emotional support etc. * Refer patients to relevant and appropriate services. * Regularly review patients and PCPs The contract value is estimated at PS140,000 per annum, with a contract term of 2 years plus 1 year optional extension with the service commencing on 1st April 2021.
