Support will be delivered by professional care staff to meet the requirements of individual support plans. This will be based on eligible needs and aimed at supporting people to be independent and to achieve their maximum potential. The service will also assist carers and parents to maintain their caring role. The service will be for people that meet national eligibility criteria for social care and people eligible for non-complex health funded Continuing Health Care at home.
