With a proactive and preventative approach to managing respiratory conditions, businesses that can offer innovative solutions, effective case management, and collaborative care models will be well-positioned to win and deliver this contract. ## Notice NHS Arden & Greater East Midlands Commissioning Support Unit (NHS Arden & GEM CSU) on behalf of NHS Southern Derbyshire Clinical Commissioning Group (SDCCG) is inviting suitably qualified and experienced providers to tender for a placed based co-ordinated respiratory service that shall provide a patient centred service that is co-ordinated and tailored to the needs and preferences of individuals, their carers and family via a proactive, preventative approach to respiratory care. The aim of the service is to proactively manage patients at risk of developing, or who have a diagnosis of respiratory disease, via the provision of a place based preventative approach to care. The objectives are to: * Support the prevention of respiratory disease, in particular Chronic Obstructive Pulmonary Disease (COPD) and pneumonia. * Support the early diagnosis of respiratory disease via active case finding, in particular COPD. * Support the accurate diagnosis of respiratory disease, in particular COPD, adult asthma and pneumonia. * Support the on-going care of patients with a confirmed diagnosis of respiratory disease via a Multi-Disciplinary Team (MDT) approach to holistic care planning, education, appropriate prescribing and co-ordination of care inclusive of pulmonary rehabilitation and home oxygen assessments. * Support the early identification, and on-going needs, of people with advanced stage respiratory disease.
