There is the potential of delivering this service for residents of Harrow and Brent also. The CCG's vision is to support non-complex/routine referrals to be dealt with in a community setting, with only complex and urgent referrals going to secondary care. By bringing the service to the community it is anticipated that patients will have rapid access to a high quality, safe and clinically effective service that is flexible and responsive to patient needs. The service will also contribute to improved clinical outcomes by reducing the time to assessment, diagnosis and treatment and by supporting increased patient understanding of their conditions to enable better self-care and management of chronic conditions. You are invited to review the attached draft Service Specification and supporting Memorandum of Information (MOI), then provide feedback to the CCG.
