The provider will provide support to patients to enable a large propoirtionto transfer to a mainstream provider of primary medical services within a 12 month period. The population covered will include: Homeless Asylum seekers Refugees Sex workers The average number of patients registered with the practice should be 1500. The registeredlist should not exceed 1750 or drop below 1250 patients, unless with prior agreement.
