The service will be provided, if needed, to patients who are registered at a GP practice in Reading and who reside within Reading Borough Council authority area. There may be a need to work collaboratively with adjacent unitary authority areas and other external partners to deliver the service. Additional information: Organisations who believe they can fulfil the requirements and would anticipate being able to provide a GP DA awareness and referral to support service should notify the council of their interest.
