## Notice

The Hospital to Home service works directly with Health & Social Care colleagues to support 3 Borough patients (Hammersmith and Fulham, Westminster and Kensington and Chelsea) discharged from both Imperial and Chelsea and Westminster Hospital. The service bridges the gap between hospital and home and prevents readmission to hospital. It supports the core priorities of the Local Authority (LA) and CCG in managing safe and timely discharge from hospital , reduction in delayed transfers of care as well as patient flow to assist the trusts Emergency Department (ED) performance. The service: * Offers short term preventative intervention on discharge following an acute hospital episode * Supports reduction in extended length of stay in an acute bed * Prevents unnecessary hospital admissions * Avoids hospital re-admissions The CCG has assessed the most appropriate route to awarding this contract, having regard to the necessity to secure continuity of these essential patient services and has concluded that the incumbent provider will assure the safest solution and being considered the only technically capable provider at this time.
