The programme is part of the system response to current and emerging national, regional and local policy for cancer services, including: * NHS 10 Year Long Term Plan * 'Achieving world class cancer outcomes: A Strategy for England 2015 - 2020 * NHS England Living with Cancer Programme * Lincolnshire Sustainability and Transformation Plan * Living with cancer Strategy for Lincolnshire 2017 - 2019, Living with Cancer Strategy for Lincolnshire 2019 - 2021 The programme has system wide scope, particularly the transition from treatment phase into 'living with cancer' (known previously as survivorship), which includes out of hospital and community based support, e.g. through local Integrated Neighbourhood Teams (multi-disciplinary teams representing commissioners, providers, local authorities, the voluntary sector and patients).
