As part of the service, the Authority is seeking innovation in the provision of integrated primary care services, which align to both the national and local strategic plan, i.e.: * Do things differently * Preventing illness, tackle health inequalities * Provide world class care for major health problems * Making better use of data and digital technology * Backing the workforce. The contract will be for a period of 10 years (with the Commissioner having the option to extend the Contract for up to a further 5 years) i.e., a maximum term of 15 years, commencing 01st April 2024. Based on the current contracts and 2023-24 Tariff levels, it is anticipated that the service provision resultant of this procurement will have an annual agreement value in the region of: * Lot 1: Earls Barton Medical Centre - PS533,752 (excl. VAT) plus PS53,375 in the first year for set up costs. * Lot 2: Penvale Park Medical Centre - PS322,256 (excl. VAT) plus PS32,256 in the first year for set up costs.
