Successful applicants will be required to work collaboratively with the CCG to provide high quality care to patients registered with a Northamptonshire GP who have been assessed as meeting the eligibility criteria for NHS continuing healthcare, NHS Funded Nursing Care or who are on a D2A pathway. The aim of the procurement will be to put in place a structure to support the purchasing of placements and to provide quality care to enable patients to meet personal outcomes as described within the service details within a tiered pricing arrangement. A range of care provision is required for the following categories of patients that will include but is not limited to: general continuing care (frail elderly), complex care (under 65 physical disabilities), those requiring dementia care and those with an acquired brain injury.
