## Summary

The procurement process for a Live-In Care Service is conducted by Hampshire County Council, aimed at providing short-term care to assist individuals discharged from hospitals within Hampshire. This service is particularly focused on supporting timely discharges, with a total contract value of £379,680. The contract period begins on 7th December 2020 and concludes on 30th September 2021. As part of a selective procurement method, this tender stage is marked as complete, with the buyer expecting all new referrals to be accepted within a two-hour response time, following COVID-19 guidelines.

This tender presents significant opportunities for businesses, particularly small and medium-sized enterprises (SMEs) and voluntary, community, and social enterprises (VCSEs) in the health and social care industry. Companies that specialise in providing flexible care services, capable of accommodating the unique needs of individuals with long-term health conditions, including those affected by dementia, are well-suited to compete. The emphasis on effective service delivery models also opens avenues for providers with innovative, responsive approaches to care management during the ongoing pandemic and beyond.

## Notice

The Interim Live in Care Service is a short-term service, aimed at providing an effective, flexible and responsive approach to discharge from the acute and community hospitals and their surrounding hospital systems for individuals who are medically stable and may require further assessment of their needs. The service will be used where a long-term package of care is likely to be needed. All service users discharged into this service will be followed up by a Hampshire County Council member of staff. This service is based on the principles of Strength Based practices and continuing enablement to maximise independence of individuals throughout Hampshire. The service's main focus will be to support the timely discharge from hospital (same day where possible) to assist that individual within their own home whilst awaiting and undertaking an assessment of someone's eligibility for social care services. The service will also provide a means for support planning to be continued and completed at home, where Reablement is not an appropriate discharge service. Following the assessment, the provider will continue to support the individual whilst a long-term package of care is being sourced and until handover to the long-term provider can be completed. Support will be offered for a maximum of six weeks. The service will be reablement focused working to achieve outcomes for the individual with the aim of maximising a person's health, wellbeing and independence to ensure a package of care that is fit for purpose is commissioned and handed over to the long-term provider. Progress towards the outcomes specified will be monitored and reviewed by both the Service Provider and Hampshire County Council member of staff working in partnership with the individual using the service. The service will cover Hampshire residents and work with all hospital systems throughout the County. As a short-term service supporting hospital discharges, the provider will be expected to accept all referrals within a 2-hour time frame as part of the COVID-19 response with service commencing within 24 to 48 hours. The Council's expectation is that the Provider will deliver effective models of service delivery that meet the needs of all individuals including those with Dementia and other long-term health conditions/requirements. Individuals discharged into this service may be COVID-19 positive and there is an expectation that the service provider will accept these individuals into this service. The Service Provider is expected to manage this need appropriately and in line with NHSE guidelines. https://www.gov.uk/government/publications/coronavirus-covid-19-providing-homecare/coronavirus-covid-19-provision-of-home-care This service is part of the system response to the COVID-19 pandemic and seasonal pressures as well as supporting usual best practice.
