Key points for consideration for interested providers are:- * Must be able to demonstrate an ability to mobilise to service provision within a short time frame. * Requires recruitment to GP positions as the two current partners are to resign and retire respectively. During the caretaking arrangement, it is envisaged that the long term future of the service will be determined through public consultation and, depending on the outcome of the consultation; a possible re-procurement will be undertaken. The practice was inspected by the CQC in December 2015 and was given an overall rating of "Requires Improvement". SDCCG and NHS England is looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets. The model will need to include provision of the following: * Essential Services * All Additional Services * Dementia & Learning Disabilities * Enhanced Services * Opening hours and service delivery from 8am - 6.30pm Monday to Friday (except statutory bank and public holidays). Due to the interim nature of this service, it is envisaged that the practice will operate under an open list and with a minimum income guarantee based on the current list size. Do you have the ability to register as a provider with the CQC at these premises by 1st April with CCG support if needed? 4. Do you have the ability to turn the practice around from one that requires improvement to a CQC rating of "good"?
