This is to provide a temporary period of assessment and enablement of people, either following a hospital admission or where there is a risk of hospital admission, who are unable to return directly to/remain at home due to requiring 24/7 assessment and enablement for a period of up to 14 days. This will be with a view to improving mental and physical wellbeing and ability, and increasing independence and confidence in order to maximise personal independence whilst enabling a holistic assessment of any on-going support needs to enable each person to return home once able, to continue their recovery in a non-residential setting. The Council is commissioning 4 bed spaces, and will continue to review demand throughout the term of the Pilot. However, there is a possibility that the requirement will increase during the term of the Pilot.
