## Summary

The South of England Procurement Centre is inviting tenders for the "Home from Hospital Service" contract, aimed at providing support to individuals medically fit for discharge from acute care. This procurement falls under the services industry sector, specifically focused on community service provision, and is located in the South East of England. The tender period is open until 28 October 2020, with a total contract value of £369,880 for a duration of two years, commencing on 1 April 2021, and an option for one 12-month extension. The procurement method is an open procedure for bids from single providers as well as consortia or partnerships, requiring a designated lead entity for the contract.

This tender presents significant opportunities for businesses specialising in community and healthcare services, particularly those offering patient support, rehabilitation, or social care services. Small and medium-sized enterprises (SMEs) and voluntary, community, and social enterprises (VCSEs) are encouraged to apply, as the approachable nature of the procurement method allows for various organisational structures to compete. By participating, businesses can expand their reach into the healthcare sector and contribute positively to the wellbeing of discharged patients, thus enhancing their service offerings and potentially increasing revenue streams.

## Notice

Further to the Invitation to Quote process halted on 20 March 2020, in response to Covid-19 pressures, North East Hampshire & Farnham CCG and Surrey Heath CCG are now republishing this requirement. The participating CCGs are working in collaboration to commission a single Home from Hospital service across both CCG footprints. The overall aim of the Home from Hospital service is to provide a responsive and time-limited service to individuals who are medically fit for discharge from acute hospital or community unit, through providing and signposting onto, advice, information and person centred support for these individuals and their carers to safely resettle at home, and to live healthy and independent lives. The Service provider(s) will link individuals with appropriate interventions (e.g. volunteers and community groups) that can facilitate support and connections for an individual and or their carer to settle back at home and restore "normal" daily functioning. It will allow people to regain their confidence after a period in hospital or after a short stay in the emergency department in a way that will promote their independence and reduce the likelihood of hospital readmission or Long Term Care - there will be prompt access which enables timely discharge. Bids from single providers, legally established consortia and/or partnerships are welcome; partnerships should note they must select a lead partner to become the legal entity on the contract. NB: This is a 2 year contract, with one extension option of 12 months.
