We aim to get people out of hospital as soon as they are medically fit and work with people to prevent further admissions. 20-hour posts available, which include a continental shift pattern that includes mornings and evenings and you should be able to travel across the borough and will therefore need to hold a full valid driving licence, have access to a car and must have business class insurance. This role requires an Enhanced Barring DBS check for Adults, also the candidates must hold a certificate in moving and handling.
