This service will provide 24/7 telemedicine clinical support to care homes in Central and West London, covering areas including Hammersmith and Fulham, Hounslow, and Ealing. Local SMEs and voluntary community sector enterprises with the necessary expertise and workforce to provide immediate clinical support and efficient referral processes would find this an advantageous project to pursue, especially as the scope may expand to include additional communities over time. ## Notice NHS Hammersmith & Fulham Clinical Commissioning Group (CCG), on behalf of the North West London Clinical Commissioning Groups comprising of NHS Central London CCG, NHS West London CCG, NHS Hounslow CCG, NHS Ealing CCG, NHS Brent CCG, NHS Harrow CCG and NHS Hillingdon CCG is seeking to appoint a provider to deliver a 24/7, 365 day per year Telemedicine-clinical support function to support care homes initially across Central London, West London, Hammersmith and Fulham, Hounslow and Ealing Clinical Commissioning Group (CWHHE CCGs) areas. This facility will be introduced to 43 residential and nursing homes across the geography covered by the CWHHE CCGs, with a phased implementation over the three years of the programme. If the service is successful, the CCGs reserve the right to extend the provision to include residential and nursing homes and patients and carers living outside care homes in the NHS Brent CCG, NHS Harrow CCG and NHS Hillingdon CCG areas. Bidders should be able to demonstrate relevant experience of delivering comparable services, and be able to evidence the capacity and workforce in place to commence provision quickly, providing immediate advice and reassurance to both clinical and non-clinical staff and if necessary, to plan for local onward referral. If successful, the service may extend to cover additional care homes in Brent, Harrow and Hillingdon CCGs as well as patients and carers living outside of nursing and residential homes in further phases. Therefore, the CCGs will work with the provider over the course of the contract to seek to increase the breadth and value of the contract to improve care for people across North West London as part of the wider Last Phase of Life Programme. It is expected that the programme will be part of the End of Life Care Incubator, which offers support from Social Finance and a dedicated EOLC Incubator Advisory Board, opportunities for sharing learning with other similar UK projects, and access to funding from the Care and Wellbeing Fund, a social investment fund that supports community-based models of health and social care. Providers will be expected to work collaboratively with the EOLC Incubator and Care and Wellbeing Fund. Additional information: The CCG anticipates awarding a 3 year contract with the option to extend for a further 2 years at their discretion. The anticipated annual contract value is PS400,000 - PS7,900,000 (subject to expansion of the programme and additional investment being agreed by the CCG).
