CYC are looking for a range of providers who have the experience, knowledge, and the appropriate registrations and trained staff to support this commission. A block commission to support hospital discharge, hospital avoidance and may also have referrals from the Hospital and Brokerage team. The contract is for an initial period of 6 months. Commencement date as soon as possible (October 2022). The value of the 6 month contract is anticipated to be in the region of PS474,194.
