## Summary

The South of England Procurement Centre is currently seeking tenders for the "Dementia Post Diagnostic Support 18.405" service, which is classified under miscellaneous health services. This opportunity is located in the South East of England and is in the open tendering stage, with a submission deadline set for 12 Noon on 29th October 2018. The contract is intended to start on 1st April 2019 and run until 30th March 2022, wherein the awarded organisation will play a crucial role in providing comprehensive support to individuals diagnosed with dementia and their carers throughout the entire dementia journey.

This tender presents significant opportunities for businesses specialising in healthcare services, particularly those focusing on supportive care and mental health. Small and medium enterprises (SMEs) and voluntary, community, and social enterprises (VCSEs) are particularly encouraged to participate, as their services might align well with the person-centred approach required for this contract. Companies that can demonstrate experience in providing emotional support, facilitating partnerships with healthcare providers, and delivering tailored care solutions will be well-suited for this opportunity.

## Notice

The aim of the Dementia Post Diagnostic Support Service is to be the main point of contact for the person diagnosed and their carer during the entire dementia journey. Potential bidders will be expected to demonstrate an awareness of and the ability to provide support at all stages (including complex, challenging and end of life/bereavement support) of the journey. The service will provide appropriate emotional support, practical guidance and information to meet the needs of those entering the service, as well as facilitating access to well-being interventions which promote independence and quality of life. The service will demonstrate a person-centred approach that is tailored to the individual needs of a variety of clinical groups. The service will be both for the person diagnosed with dementia and their family carer/supporter. The service will also provide support for those people who at the end of the diagnostic process are felt by diagnosing clinicians to have Mild Cognitive Impairment (MCI). The service will provide support for those people under 65, who have a diagnosis of early onset dementia. The service will work in partnership with a wide range of organisations including General Practitioners as part of an integrated approach. It will facilitate identifying issues leading to deterioration and will work with agencies to ensure a timely and pro-active response. It is expected that Care Coordination and support for Advanced Care Planning will assume a key element of the Dementia Post Diagnostic Support Service. It is also expected that the preferred provider will demonstrate the ability to support the local Memory Service (where appropriate) to achieve compliance with Nice Guidance and local Primary Care providers (again where appropriate) to deliver objectives in Locally Commissioned Service agreements. Additional information: The Services to which this ITT relates fall within Chapter 3, Section 7, Particular Procurement Regimes to the Public Contracts Regulations 2015 and Part H of the Annex V to the Public Contracts Directive (also known as "light touch regime"). The deadline for submission of completed ITT responses is 12 Noon on Monday 29th October 2018. Should Bidders have any queries, or are having problems using the portal contact In-tend on 0845 557 8079 Monday to Friday (9am to 5pm) or email support@in-tend.co.uk
