The model is comprised of 3 integrated pathways (detailed below) accessed through a single point of access. * Single Point of Access * Assessment Pathway * Access to Housing Pathway - Main Provision - Specialist Provision * Housing Sustainment Pathway. The scope for this procurement is the Complex Needs Service which forms part of the Specialist Provision Pathway. Key aims of the Service will be: * To promote independent living skills and increase the service users ability to manage and maintain a tenancy. * To focus on harm reduction, recovery and resilience * To support service users' to address their drug and alcohol issues by working in partnership with appropriate specialist support services. * To prevent re-offending * To create opportunities for change and progression * To promote positive approaches to health and wellbeing. The service will support people living in 8 units of accommodation at Springhill Close in Outwood, Wakefield. The duration of the contract detailed is for the initial term of the contract.
