This involves more collaborative working between key stakeholders with GPs, health and social care professionals and non-traditional provider (NTP) organisations coming together to organise person centred care more effectively. We also have a particular focus on supporting people to access support in the community in order to help them define and meet their goals. We are seeking to build on the experience of delivering a navigation service over the course of the last five years and support the development of Islington's personalisation offer.
