It specifies services to be provided to Working Age Adults who are eligible for Adult Social Care who are living in a supported living service for client-specific requirements for the provision of accommodation based care and support and housing support for: People with learning disabilities People with mental health support needs; People with physical disabilities, sensory impairment and long-term conditions. The Council wishes to work in partnership with Service Providers in delivering a high quality of support to its clients and hopes to maximise the use of available resources by establishing longer-term, more integrated relationships through a contractual framework which is flexible enough to reflect changing needs, priorities and lessons learnt, and which encourages client participation.
