The service has four core objectives 1. Deliver a service that supports adults to adopt healthier behaviours and to set and achieve personalised and realistic goals regarding their weight and overall health (As per the specification for the Holistic Adult Weight Management Service in section 4 which includes sub-contracting responsibilities). 2. Temporarily support any remaining service users on the adult Tier 2+ pathway (As per section 5) 3. Deliver a service that supports children, young people and their families to adopt healthier behaviours and to set and achieve personalised and realistic goals regarding their weight and overall health (As per the specification for the Holistic Family Weight Management Service in section 6). This includes delivering a family weight management service, pro-active feedback calls to families with child above the 99.6th centile and NCMP linked proactive work in a small selection of schools. 4.
