Some of the elements of this model are being procured as part of this contract. The contract has been split into the following 3 lots: Lot 1: Assessment Pathway The Assessment Pathway will comprise of: * Preventing Rough Sleeping Service * Assessment Centre * Short term Supported Housing Lot 2: Access to Housing Pathway - Main Provision The Access to Housing Pathway will provide housing related support to people who are at risk of homelessness & need support to both find & maintain suitable housing. The Pathway will support people to find & access accommodation & then provide outcome-focussed housing support in order to enable them to gain the skills to live independently. Lot 3: Housing Sustainment Pathway The Housing Sustainment Pathway will provide housing related support to people who are at risk of homelessness to enable them to keep & maintain their housing. Bidders may bid for one, some or all of the lots available, however the Council reserves the right to only award one lot to a bidder.
