## Notice

Services will be offered from three mandated sites which will be geographically distributed across the three localities of the borough: *South West Locality: Gracefield Gardens Health and Social Care Centre, Streatham, London SW16. *South East Locality: West Norwood Health and Leisure Centre, 25 Devane Way, West Norwood, London SE27 0DF. *North Locality: Springfield Medical Centre, 110 Union Road, Stockwell, London SW8 2SH
