The service will work across health and social care to support service users to access services which can meet their needs in a timely and effective manner. This is anticipated to include: - Effectively connect with, support and coordinate high intensity ambulance callers and A&E attenders within the SWBCCG footprint - Reduce the activity High Intensity Users have in GP practices and non-elective admissions - Establish, utilise and coordinate connections with multi-agency and existing professional services to negotiate a new and innovative way forward - Demonstrate a reduced workload on unscheduled care services and the wider health economy resulting from reduced 999 calls, which otherwise would have attended A&E, result in an admission, or a call to the police - Safely manage and coordinate the needs of the client group through the use of robust community and service connections and the voluntary sector - Support patient choice and compliance - Provide fertile commissioning intelligence across all providers and in doing so, lower the stigma associated with high intensity users - Create and coordinate a replicable service which can be integrated and managed over the longer term across other providers.
