There are no conditions relating to branding and publicity, audit and eligibility information will be collated from the quarterly reports forming part of the monitoring of the service. The information required for the annual report is set by DLUCH and the provider would be expected to report on all information required where relevant and be aware that this may change in line with any changes made by DLUCH.
