## Notice

North Staffordshire and Stoke on Trent CCGs wish to commission a community based falls coordinator service for the North Staffs and Stoke on Trent CCG population. The community based falls service will be expected to reduce individuals falls risks, improve patients' health outcomes and support people to live independently for longer. To minimize the harm due to falls and the number of associated hospital admissions there is a requirement to focus on prevention. An effective community falls service will work with a range of organisations to ensure that patients at risk of falling are identified, preventative interventions are delivered and that for those that do fall, multifactorial care plans are in place. North Staffs and Stoke-on-Trent CCG have been highlighted as poor performers in relation to falls related admissions, as defined in the NHS England Improvement and Assessment Framework 1 The service will be expected to reduce the number of falls related admissions to hospital and by working collaboratively with other services, will support a reduction in costs associated with treatments for hip and other fragility fractures. The CCGs will be holding a potential bidders event on Wednesday 30th November, 12 - 1pm; to provide more information about the objectives of the service, clarify procurement timelines and allow organisations to ask further questions Additional information: If and when this requirement is offered to tender, this will be done via electronic means using the internet. Midlands and Lancashire CSU is conducting this procurement exercise as a central purchasing body for another NHS body with whom the successful bidder(s) will enter into contracts for the supply of the services. Any other public sector body wishing to access the contract may do so only with permission from the contracting NHS body. The contract conditions will be set out in the Invitation to Tender.
