The purpose of this questionnaire is to give providers the opportunity to help inform and understand how the CCG can approach some of the key issues associated with the delivery of the described services. The outcome of this is intended to help to inform the CCGs how to approach future opportunities for the services to ensure that the best outcomes for service users are delivered. The CCG requests that questionnaire responses are submitted to scwcsu.clinical.procurement@nhs.net by 5pm Friday 11th August 2017.
