Rapid Response will also prevent the admission into hospital of service users who are medically fit, but who require additional support to remain at home. The Service provided is only for Adults and Older People who have been assessed as having eligible needs under the Care Act 2014 and the Care and Support (Eligibility Criteria) Regulations 2014 and who are ordinarily resident within the Cheshire East area.
