Total funding for contract PS107,353.75.Home from Hospital service operates as planned interventions and works in partnership with other service providers to prevent hospital admission through a range of support and interventions and where hospitalised, minimise disruption and ease the adjustment following a crisis or hospital stay, both for the patient and their formal carers. The range of services can be varied based on their local circumstances and the need of the individual Additional information: The contracting authority considers that this contract may be suitable for economic operators that are small or medium enterprises (SMEs). However, any selection of tenderers will be based solely on the criteria set out for the procurement.
