## Notice

The service will facilitate the safe transfer of care for service users who are medically fit for discharge from acute mental health inpatient units. It will be available for 5 people at any point in time. The focus will be to support access to suitable accommodation and assistance to maintain a tenancy. The service will be provided for a maximum period of 28 days for service users meeting the eligibility criteria; any exception to the length of stay will be by mutual agreement between the provider and commissioner and reported as exception in the quarterly performance report. Value of contract: Maximum Contract value 110,800 per annum Term: 4 years with 1 year with possible extension How to Express Interest in this Tender: 1. You can now use the 'Correspondence' function to communicate with the buyer and seek any clarification (any clarifications will be found within the 'Clarifications' tab). - Note the deadline for completion, then follow the onscreen instructions to complete and submit your response. - There may be a mixture of online and offline actions for you to perform (there is detailed online help available). You must then submit your reply using the 'Submit Response' button at the bottom of the page.
