## Notice

Hampshire County Council invites applications from suitably experienced organisations who wish to be considered for selection to tender for provision of OT assessments outsourcing. The overarching aim of the service will be to: * Provide an occupational therapy assessment, diagnosis & support service to a predefined number of referred non-urgent cases. * Provide assessment for equipment & minor adaptations as identified * Support DFG process management including assessment processes & specifications as required, directly liaising with District & Borough Council housing & Grants teams on behalf of HCC Reablement. The provider will carry out person centred occupational therapy functional assessments in accordance with Care Act 2014 principles and in line with Hampshire County Council policies. Where identified and practical to do so the provider will facilitate meeting the determined outcomes of the assessment with the support of the Hampshire County Council's Adult Health & Care Services Reablement Teams in line with relevant HCC eligibility criteria's where they apply. Interested parties should register their interest in the project via the In-Tend website https://in-tendhost.co.uk/hampshire, and complete and submit the tender return document together with any supporting information requested by 12.00 hrs on Friday 8th October 2021.
