## Notice

It is a contractual requirement that the clinical service will operate within the Middlesbrough ACT (Accessing Care Together) partnership model to provide a single, overall service with a shared identity and branding, to deliver the integrated system. This service will work in partnership with other support services - both within the ACT model and also with relevant partners throughout Middlesbrough - to promote positive change in people's lives, by having effective pathways and a strong ethos of partnership working in place.
