## Notice

The service will provide individualised sub-acute healthcare to patients in their own home who are not medically fit for discharge, but who are clinically stable and can have their care delivered in a less acute setting. Patients remain under the care of their hospital Consultant whilst receiving the CARE at Home Service (CAHS). A care plan and estimated discharge date are agreed with the KGH Consultant team prior to the patient transferring home. The provider will liaise with the referring team should the care episode need to be extended beyond the estimated discharge date.
