The Making Connections Walsall Programme is targeted at meeting the social needs of the older adults who are lonely and socially isolated NOT SOCIAL CARE or MEDICAL needs. Providers with a local understanding of Walsall's Older population and experience of successfully supporting vulnerable older people to address their social needs and access community initiatives.
