## Notice

The service will support people who may not be eligible for social care / NHS support but whose needs are likely to escalate without any intervention. It will ensure vulnerable people are supported with initial practical support for up to 6 weeks to promote confidence and independence. The service will support people through a range of interventions, including: * Undertaking simple shopping * Ensuring the home is warm and basic provisions are available * Arranging for key safes to be fitted and lifeline systems to be provided * Support with paying bills and making appointments * Collecting medication prescriptions * Support to maintain and improve social networks * Support with advanced care planning beyond initial short term support
