This service will enable residents to take responsibility and control of their own care and enhance the collaboration between the Local Authority and Health services at a local level. The provider will need to demonstrate that they have the knowledge, innovation and ability to deliver the services that meet the needs of a diverse population. Contract start date is the 1st April 2020 and will be for a term of three years with a possible extension of one year, plus one year. The financial envelope available for this service is PS720,000 per annum.
