NHS Leicester City CCG, NHS West Leicestershire CCG and NHS East Leicestershire & Rutland CCG (together known as the Leicester, Leicestershire and Rutland CCGs, or LLR CCG) NHS Midlands & Lancashire CSU (MLCSU) on behalf of the Commissioners (the LLR CCGs) is publishing their intent with regards to the following service: The CCG's are looking to commission a Type 2 Diabetes Structured Education Service and look forward to receiving bids from providers that can deliver this service which has come to an end of its original contract. The aim of the service is to support adults aged 18 years and over registered with a GP in Leicester, Leicestershire and Rutland areas diagnosed with Type 2 Diabetes Mellitus to improve knowledge and self-management of their condition through the provision of a diabetes education programme. The programme will be offered to all patients with a diagnosis of Type 2 diabetes with priority given to those that are newly diagnosed (newly diagnosed being up to 12 months from the date of diagnosis) or those not achieving NDA targets (BP
