Dear Provider, We would like to invite you to an online consultation session on 16th August 10 -12am where we will be seeking your views on the service specification for the upcoming Community Health and Wellbeing Service (CHWS) pilot. The pilot is to trial a new way of delivering home care type support in one area of the city from April 2024. This will involve bringing together Home Care, health care, and local community support services to provide a more consistent and joined up support package that's centred around the needs, strengths, and assets of our residents. A draft of the service specification will be circulated shortly. We are expecting lots of interest in this event so we will need to limit it to one representative per organisation - thank you for your understanding.
