The desired outcomes for individuals (Clients and Carers) are that: People are supported to be as independent as possible People have access to timely and responsive care People are supported by skilled staff, delivering holistic and personalised care People are kept safe from avoidable harm People are able to live well People are respected and able to make informed choices about services.
