## Notice

London Borough of Newham (LBN) and Newham Clinical Commissioning Group (NCCG) are carrying out a market engagement exercise on the proposal to commission an integrated health and wellbeing service (IHWS). We are seeking the views of potential providers on the proposed model which brings together weight management, physical activity and stop smoking services as described below. Please note that this is a market test and not a formal invitation to tender. Individuals who manage their lifestyles are healthier, more productive, have fewer absences from work, and make fewer demands for medical and social services. Early intervention and prevention, keeping people healthy and out of acute and urgent services, has long term health and wellbeing advantages for residents and efficient health and social care services. Wellbeing in this context is defined as a proactive, preventive approach that emphasises the whole person and which works to achieve optimum levels of physical, mental, social and emotional health. It is anticipated that the IHWS would commence 1.4.2021 with an initial term of 3 years with the option to extend for a further 2 years. Budget is to be confirmed, but the ambition is to allocate up to 1 200 000 GBP per annum to run the service, which would be funded jointly by LBN and NCCG based on projected activity levels. It is likely the procurement process will be led by LBN and will commence spring 2020. It is anticipated that the service will develop incrementally over the course of 3-5 years and will therefore be commissioned on this basis, in order to be responsive to opportunities as they emerge. Additional information: The service specification will be developed in partnership with a range of stakeholders including residents and service users but is likely to include: - a person centred and strength based, integrated, single point of access (SPoA) gateway/'triage' function to provide a holistic assessment of individuals. -- this will identify needs and priorities, prompt and support sustainable effective behaviour change using effective recognised behaviour change techniques; -- monitoring and communication of progress to clients and appropriate relevant health and care professionals (eg GPs, social prescribers, social workers) to ensure clients get the most appropriate and effective intervention(s) and realise their goals and outcomes. - diet and nutrition education/advice including diabetes remission and prevention, - structured physical activity programmes, - weight management programmes (for adults and children/families) including diabetes remission and prevention, - tobacco consumption and smoking cessation service. The service provider will be required to: - provider leadership and support for new and existing community asset development related to universal services i.e. those not directly commissioned as part of the lifestyle services, -- for example, social prescribing assets, community initiatives and volunteer based programmes e.g. physical activity initiatives such as walking groups or community sessions; -- develop effective relationships and pathways with other services and organisations to provide an integrated approach to wellbeing. - i.e. other preventative/treatment services such as Well Newham (social prescribing) mental health; sexual health; drugs and alcohol, primary healthcare services (e.g. GPs), social health services (e.g. debt, housing and benefits crisis services), - provide advice and signposting in relation to the wider determinants of health, including; debt, benefits, welfare, training and education, - proactively tackle health inequalities through working effectively with a range of diverse communities where the needs are greatest and the health is poorest and operating a model based on priority need for the individual referred, - promote self-help, for those who are able to self-support, - carry out marketing for the service including the delivery of wider health promotion campaigns (national and local) and training frontline staff in brief interventions. We would welcome views on models of delivery; specifically the benefits of the service being delivered by a collaboration of different providers as part of an alliance or commissioning all or some elements of the IHWS together.
