The commissioners would like to hear from any potential provider who would be willing to complete and submit the attached form, and then present and discuss their probable service delivery model, capability and capacity to help design the LLR service specification. If you already submitted the attached form in response to the previous advertisement you do not need to resubmit this, but you will need to contact us on the details below to arrange an appointment. The commissioners would value expert insight that can be given into the delivery of this type of service and information would be greatly received. Whilst this is to help properly design a realistic service specification for an upcoming tender covering the LLR region the information you give may be shared with other CCG's in England.
