Collection will be from Prospect Park Hospital Pharmacy. To be delivered to the patients address, care home or clinic. This agreement will be for the use of Berkshire Healthcare NHS Foundation Trust. The contract duration will be for 2 years, with the option to extend for a further 1+1 years.
