It is being led by SMBC Adult Social Care (ASC) as part of the 'Stockport Together' neighbourhood plan to provide additional home support capacity across the borough. It will work in close partnership with the 'REaCH' in -house service to identify individuals who can benefit most readily from a re-ablement approach.
