## Summary

The East Sussex County Council, through its Adult Social Care and Health Department, has awarded a contract for the provision of Hospital Discharge Services effective from 1st October 2019, lasting for 5.5 years until 31st March 2025, with a potential 24-month extension option to March 2027. This procurement, classified under health and social work services, was initiated using an open tender procedure, and the total contract value is approximately £2,262,405. The project aims to enhance the wellbeing and independence of individuals within the community, focusing on reducing the need for formal health and social care services. The services are split into two lots: assisted hospital discharge and home from hospital services, with delivery expected predominantly in East Sussex (postal code BN7 1UE).

This tender represents a significant opportunity for small to medium-sized enterprises (SMEs) and voluntary, community, and social enterprises (VCSEs) specialising in health care and social services. Companies that focus on patient care, rehabilitation, or community support are particularly well-suited to compete for this contract, as the council seeks providers capable of fostering independence and improving health outcomes for residents. Interested providers are encouraged to explore the detailed service requirements outlined in the service specification documents.
