## Notice

This will include completion of assessments for adults and children. The assessment will be fully documented and populated on the Council's Social Care system along with case notes within the set target time. Recommendations made in the assessments, such as onward referrals or the provision of equipment will be progressed by the Provider and the case held until completion.
